Release Forms



Player Medical Release

This form allows the authorized adult team personnel to release this information in the event of a medical emergency to a third-party medical provider.



Permission/Liability & Photo Release Form

This form acknowledges that Central Valley Volleyball can utilize any photos taken during the season and that you assume all risks and hazards incidental to such participation.

AAU Membership is required. The cost is $16 for regular membership. Follow the link above, and click on “Join AAU”, then “athlete membership”. CVVC’s Club Code is W3E4C5. A signed copy of your AAU card is required to be kept on file with your coach.


